In Focus:
The Stories of Three Grantee-Partners Working to Transform Rural Health
In 2024, as part of our Transform Rural Health campaign, we awarded unrestricted grants to three nonprofit organizations: Finger Lakes Community Health, based in Sodus, NY; Providence Farm Collective, based in Orchard Park, NY; and Refugee and Immigrant Self-Empowerment, based in Syracuse and Buffalo, NY. All three were chosen for their commitment to improving the health of their rural communities and advancing health equity. The grants, totaling $200,000, were made possible by a 2022 gift to the Health Foundation from billionaire philanthropist MacKenzie Scott.
In addition to the funding, each organization was able to partner with the Health Foundation to produce a short video about their mission and impact.

Providence Farm Collective (PFC)
Operating 37 acres of farmland in Orchard Park, NY, Providence Farm Collective provides entrepreneurial opportunities for refugees and immigrants with sustainable farming skills. The goal is to transform the production and delivery of healthy organic food, ensuring equitable access for PFC families and local communities.
Finger Lakes Community Health (FLCH)
“Taking the time to care.” That’s the motto of Finger Lakes Community Health, a Federally Qualified Health Center with 11 rural locations throughout New York’s Finger Lakes region. Committed to health equity, FLCH serves more than 28,000 patients a year and provides comprehensive, personalized care to anyone who needs it—regardless of their ability to pay.
Refugee and Immigrant Self-Empowerment (RISE)
Through diverse programs and services, RISE provides immigrants and refugees with access to fresh organic food and quality health care, and the tools for economic self-sufficiency. With the goal of becoming a one-stop shop for New Americans, RISE offers transportation services, care management, and immigration support.
